If you are eligible (Check the practice area), you can register at the practice by filling in a purple GMS1 form and calling in at the surgery.

If you have an NHS medical card or anything that has your NHS number on it please bring that also.

 

In addition to this we will also need to see proof of identity and address. You will also need to agree and sign our 'Acceptable Behavior Agreement'

if you are moving from a different surgery, then your record will not be available to us for a short time and therefore we ask you to provide some information about yourself on our 'New Patient Questionnaire'.

If you prefer then you can download and print these forms out and bring them with you already completed to the surgery.

Patients have the right to express a preference for a particular practitioner and wherever possible these needs will be met.
